How to file a complaint
You may file a signed, written complaint 180 days to the date of the alleged discrimination. The complaint should include:
- Your name, address and telphone number.
- The name, address of the agency, institution, or department you believed discriminated against you.
- How, why, and when you believe you were discriminated against. Include as much specific, detailed information as possible about the alleged acts of discrimination, and any other relevant information.
- The names of any persons, if known, who Southwest Human Resource Agency could contact for clarity of your allegations.

United Way CampaignIt is United Way Campaign during the month of FEBRUARY.
Employees can contribute by giving any amount and designate where they would like the donation to be put.
Any employee who contributes an hour a month of their pay to United Way gets a day off during the year.
If you are currently contributing, you only need to complete a form if you want to make a change or stop your contribution.
If you do not complete a form and you are contributing an hour a month for the United Way Day your hourly amount will be updated beginning the first check in April.
All COMPLETED UNITED WAY FORMS will be due by February 28, 2019.
